PRIVACY PRACTICES NOTICE

The following Notice explains how medical information about you is utilized and disclosed and how you may get access to this information if you so chose. Each time you visit the Yuba College Health Center, a record of your visit may be developed. This record contains information that can identify you and may also contain financial information. Other information including, but not limited to, are your symptoms, diagnosis, medical notes, test results and a treatment plan will be in your medical records. The Protected Health Information that is contained in a medical record is the physical property of Yuba College.
